Proof of past four weeks of income, if working
Proof of any other health insurance

BE PREPARED TO PROVE IDENTITY AND CITIZENSHIP TO THE ISD OFFICE BY SHOWING ANY OF THE FOLLOWING DOCUMENTS:

A current state driver's license with a picture
A state-issued identity card with a picture for a non-driver
Children who are age 16 or younger may provide one of the following: a school identification card with photo; a military dependent's identification card with photo; a school record that shows a date and place of birth and parent(s) name; clinic, doctor or hospital record showing date of birth; or daycare or nursery school record showing date and place of birth

CITIZENSHIP

An original U.S. birth certificate
A certification of birth issued by the Department of State
A U.S. hospital record made at the time of birth, created at least five years before the initial application date and indicating a U.S. place of birth
Institutional admission papers from a nursing home, skilled nursing care facility or other institution, created at least five years before the initial application date and indicating a U.S. place of birth

People born outside of the U.S. must show a certificate of birth from the country they were born.
